Parental Consent (Treatment of a Minor)
Parental consent is required for students under the age of 18 to be treated for health-related services.
The Center for Well-Being Health Services provides high-quality, patient-centered health care that promotes the health and well-being of the entire St. Thomas community.
Our health staff includes primary care physicians, psychiatrists, nurse practitioners, registered nurses, physical therapists, certified medical assistants, a registered dietitian, and a health case manager. Our services include Primary Care, Psychiatric Care, Preventative Care, Women's Health, Physical Therapy, Travel Clinic, Urgent Care and Wellness Services. We work closely with Counseling and Psychological Services, Health Promotion and Athletics.
All visits and personal information are confidential. All medical records are confidential and can only be released with your written permission.

The Center for Well-Being strives to provide compassionate, high-quality services in a caring environment that is affirming of all sexual orientations and gender identities.
Students can include chosen name, gender identity, and sex assigned at birth in medical records. Health Services will honor your gender identity but will use your sex assigned at birth for legal medical documentation and treatment.
Our electronic health record system utilizes terms such as preferred name and preferred pronouns, we recognize that the names and pronouns we use are not just "preferred," they're our names and pronouns.
We encourage LGBTQIA+ students to receive routine care. Specifically, we recommend that students follow guidelines for routine screening of sex-specific organs, like pap smears/gynecological exams, breast exams, prostate exams, etc. If you are uncomfortable with these types of exams, we are happy to talk with you about how to make them easier.

Request that your records to be sent from University of St. Thomas Health Services to another provider or grant permission for records to be shared with a parent/guardian.

Allergy shots may be administered by St. Thomas Health Services with some stipulations. Use this form to provide consent.
