Immunization Requirements
The state of Minnesota requires all undergraduate students born after 1956 to be immunized against tetanus and diphtheria (TD), and measles, mumps and rubella (MMR).
If we are unable to verify your immunization dates through the Minnesota Department of Health immunization registry, you will be contacted to provide your immunization information. If you do not receive an email or secure message, no action is needed.
Deadline: Friday, October 18, 2024
A Registration Hold will be placed on students who have not fulfilled immunization requirements by the deadline.
To provide your immunization information:
- Log in to myHealthPortal using your St. Thomas credentials
- Select Forms & Requirements
- Select Update for Measles, Mumps, Rubella or Tetanus-Diphtheria
- Under Vaccine History, select Add Immunization and enter date and vaccination type
- Repeat for additional vaccinations (two MMR dates are required)
- When entering the MMR vaccine for Measles, the date will also be added for Mumps and Rubella
- If the dates provided meet requirements, the item will be marked as “Compliant” in Forms & Requirements
- Before exiting myHealthPortal, ensure the Forms & Requirements page shows “Compliant” for:
- Measles
- Mumps
- Rubella
- Tetanus-Diphtheria
You are not required to upload a copy of your immunization records but may choose to do so to assist in clinical care and/or travel health.

Immunity, Exemptions and Records
If you have had Measles, Mumps, or Rubella, you may provide results of a titer blood test to prove immunity or a physician's signed document attesting to either laboratory evidence of immunity or past history of the disease.
There is no option for proof of immunity to tetanus-diphtheria (TD). Vaccination is required.
- Log in to myHealthPortal using your St. Thomas credentials
- Select Forms & Requirements
- Select Update for the correlating disease (Measles, Mumps, or Rubella)
- Select Add Test Result
- Enter Date, Test, and Result
- Alternatively, a healthcare provider can attest to laboratory evidence of immunity or past history of disease

If you wish to request a medical or conscientious/religious exemption:
- Log in to myHealthPortal
- Select Other Form Uploads from the menu
- Download the Exemption Form
- Medical exemptions must be signed by your medical provider
- Conscientious/Religious Exemptions require a notarized signature
- Return to Upload the completed form
- If documentation does not fulfill requirements, the student will be sent a secure message via myHealthPortal
Your family or health care provider may already have this information or the health office of your previous
school. International students may have an immunization booklet.
Online resources may also be available from the state in which your vaccinations were received:
- Minnesota - Health Services will check the Minnesota Immunization Information Connection for your records.
- Wisconsin - See the Wisconsin Immunization Registry.
- Iowa - See the Immunization Registry Information System.
- Other states - See the CDC Contacts for IIS Immunization Records to see if the state in which you received your vaccinations has an online immunization information system.

Graduate & Professional Students
Graduate and professional program students are encouraged to provide their immunization information to assist in clinical care and/or travel health, but it is not a requirement. To upload your records, select Immunization Record under "Additional items NOT required for clearance" on the Forms & Requirements page in myHealthPortal.
