CADI Case Manager/Service Coordinator
 
Qualification:
-One year experience in the delivery of social service to adults with mental illness or brain injury
-Experience working with adults with mental health diagnosis, homelessness, or complicated medical conditions
Responsibilities:
-Develop community support plans
-Inform person or legal guardian of service options
-Assist person to access services, develop service agreements and document through case notes
-Comply with MN Health Care Program

CHILD PROTECTION SOCIAL WORKER

Job Description:
•    Intake CPSWs obtain and assess information from reports made by phone, email, fax, or in person to determine whether the report constitutes alleged child maltreatment as defined by the State Department of Human Services.
•    Investigation CPSWs conduct investigations/assessments of alleged maltreatment.
•    Case Management CPSWs provide direct social work services to families in which abuse or neglect has been reported in order to carry out the statutory mandate to investigate, protect children from further harm, and safeguard and enhance the welfare of abused/neglected children.
