Follow-up Paperwork: Short-Term Programs 

You are required to complete a medical report prior to participating on a short-term program abroad. This form is given to the faculty director(s) should you have a medical emergency.

If your course offers multiple levels, you required to complete a Request for Registration form. Completion of this form will ensure that you will be registered for the appropriate course. If your course is not listed on this form, you do not have to complete it.

Program fees are based on group travel arrangements. Thus, if you would like to change your flight a service charge will be levied for all flight deviations (with the exception of connecting domestic flights to/from Minneapolis-St. Paul International). See the Flight Deviation form for specific details.
