2008-2009 Academic Year Undergraduate students enrolled in 12 or more credits and graduate, professional or law students enrolled in 9 or more credits are required to demonstrate health insurance coverage every fall and spring semester. Students who meet this requirement and do not complete the health insurance verification process by the 10th day of classes will automatically be enrolled in and billed for the UST-sponsored plan.Students who do not meet the credit requirement for for health insurance may voluntarily purchase the UST-sponsored health insurance plan during the enrollment periods. Domestic students must enter their information via Murphy Online at https://banner.stthomas.edu. If you were insured under the 2007-2008 UST-sponsored insurance plan and wish to continue with no lapse in coverage, you must sign up through Murphy Online by September 14, 2008. A list of health care providers for the UST-Sponsored health insurance plan can be found at www.prefferedone.com Claim form for UST-Sponsored Insurance Plan Claim forms can be downloaded from the University of St. Thomas Student Health Service website and submitted along with receipts to the address on the form. Bills for which benefits are to be paid must be submitted within ninety (90) days from the date treatment was provided.
