2009-2010 Meal Plan Change Request Form
Meal Plan Requirement
All students residing in a University Residence Hall are required to take an approved meal plan for resident students provided weekly except during Thanksgiving, Christmas, mid-semester, and Easter recesses. All students residing in Morrison Hall or Flynn Hall apartment who are not at least Sophomore class status must take one of the approved meal plans.
Dining Services provides meals to the students beginning with dinner the day before classes begin and concluding with lunch on the last day of finals or before a recess. January Term meals are covered by a separate board agreement. The dining service contract provides for food consumption in or on University premises. Any special circumstances must be arranged through Dining Service. A student is not entitled to a credit or refund from meals not eaten at the Dining Service and meal identification may be used only by the student to whom it originally is issued. A student who permits another person to use their meal identification is subject to penalties which include, but are not limited to, a fine and loss of Dining Service privileges without any refund of any fees already paid.
January Term Meal PlanStudents residing in a University Residence Hall during January Term are not required to take a meal plan. However, several meal plans are available and listed on the current academic Housing and Dining Service Rates link at the left. January term meal plan applications are available at the Card Office, 259 Murray Herrick Campus Center.
Meal Plan ChangesAll resident students may change their choice of meal plans to a lower plan, by completing a Dining Service Change Request Form by the second Friday after the semester has started. You can raise your meal plan at any time. This change does not alter the student's responsibilities as discussed in the Student Housing and Dining Service Agreement.
The meal plan choices indicated on the Meal Plan Change Request Form will be effective for the remainder of the academic year in which it is signed unless a more current change is requested and approved. The student's account will be adjusted for a prorated amount of the change requested. Print, complete and return the form to the Residence Life office, 106 Koch Commons. Forms are also available in the office.
