Instructions

1) Please complete all fields below and be as specific as possible. You must also enter your email address.  Each issue will be answered in order of date received.

2) All changes must have the approval of the Chairperson or Director of the Program.

3) When the form is complete, please click on the submit button at the bottom of the page. This form will be sent directly to the Registrar's Office and you will also receive a copy of the information that you submitted on the form.

4) You will receive a follow-up e-mail within 10 business days, detailing the action that has been taken.
