Instructions: 
1) Please complete all fields below. Allow approximately 10 business days to process this request.

2) When the form is complete, please click on the submit button at the bottom of the page. You will receive confirmation by e-mail when your attribute has been created.

3) Questions regarding attributes should be directed to mjglirbas@stthomas.edu for undergraduate areas or tltopness@stthomas.edu for graduate areas.

Important notes:

1) Effective term = the term in which the attribute should become effective. 

2) If you need to add a new attribute, please use the comment field at the bottom of the form.
