Supervisor’s request for supervisee access to Banner student system forms controlled by the university registrar.

Please complete and submit this form to the university registrar’s office.   Is this access for a new employee to the University of St. Thomas?  If YES, they also need to complete the online FERPA COURSE AND QUIZ before access may be granted.  Also, if the employee is not new, but has never completed the online FERPA course and quiz, they will need to complete that before access will be granted.  Please contact Linda Dorn at ljdorn@stthomas.edu with the name(s) of your employee(s).  She will email your employee(s) the instructions to access the course and quiz.  Please complete this form after they have completed the online course and quiz.

Requesting Access

Following are Banner system security classes for which access is controlled by the university registrar.  For each security class the supervisor must indicate whether the employee requires maintenance (update) access or query only access.  Maintenance access cannot be granted until the employee has successfully completed certified training on the use of the form(s).  Maintenance access permits the employee to enter or change data on the form.  The completion date for the required training must be entered for security class for which maintenance access is requested.

Access to any of these forms automatically includes query access to the Banner General Class which includes query access to SOAIDNS, SOAIDEN, SPAIDEN, SPATELE, GOAEMAL. These are search forms, and include a person's address, telephone number and email address.

Please note: A person with maintenance access can access SGASTDN and can change programs, majors, minors, and catalog term.  They can also add and delete advisors and add and expire holds. For maintenance access, contact Linda Dorn.  A person with query access can look at all of these items, but cannot make changes. Both maintenance and query access requests include view only access to past course work.

Please note:  Only Graduate and Law level Department Assistants or Coordinators are allowed maintenance access.  All undergraduate maintenance access is centralized in the Registrar's office.  Catalog allows a person to create a new course template in Banner.  Proper approvals from the GPPC and UPPC must be received before the appropriate party may enter these courses. A person with query access can look at all of these items, but cannot make changes.
