How do I get a copy of my W-2 tax form for previous years?
a. You can access your W-2 Year End Earnings Statement for all previous years
going back to 2003 via Murphy Online using the following instructions:
• The link to Murphy Online is MURPHY Secure Login.
To log in use your UST ID number and PIN Number.
• Once you are logged in to Murphy Online, click on Employee Services.
• Select Tax Forms from the Employee Services menu.
• Select W-2 Year End Earnings Statement from the Tax Form menu.
• Select the Year W-2 Menu and click on the Display button.
• On the W-2 screen, click on the Print Button at the bottom for a printable
copy make sure your page setup under the File button is on Landscape to

copy of the W-2 using the Request for Duplicate W-2 form and send it to
Payroll at AQU 202 or email your request to firstname.lastname@example.org.
How do I change my tax withholding?
a. Tax is withheld in accordance to the tax status you claim on your W-4 form.
b. An employee may change the exemptions they claim on their W-4 form at any
time and as often as they want.
c. The W-4 form may be completed online but must be printed, signed and then sent to
the Payroll office (AQU 202). It may also be faxed or scanned and emailed.
What should I claim on my form W-4 Employee's Withholding Allowance Certificate?
a. The number of exemptions you claim on your W-4 form is different for each

form only your tax accountant can do that.

When are the W-2 annual tax statements sent?
W-2 tax statements are mailed once a year to the employee’s home address by no later than
January 31st of the following year. You can also access a copy of your W-2 via Murphy Online
at approximately the same time.
What do I do when my citizen status or Visa type changes?
When an international employees becomes a citizen they must notify Human Resources and Payroll immediately. A new W-4 form needs to be processed for Payroll and the I9 form needs to be updated for Human Resources.
When an international employee changes their VISA type, they should notify Payroll. This may or may not effect their tax status but it is always best to confirm this with the Payroll department.
How do I update my W-2 mailing address?
If you have moved, it is important that you update your address on Murphy Online. The deadline to update addresses on Murphy Online to reflect where your W-2’s will be mailed is Friday, January 8th. If you do not update your address by January 8th, your W-2 will be mailed to the incorrect address. It is your responsibility to make sure your addresses are up to date on Murphy Online.
When logging into Murphy Online, please be sure to update your Home Primary Residence to your current address. This is the address your W-2’s will be mailed to. If your pay check or direct deposit goes to your home address, please be sure to also update the Payroll Check Mail Address as well. W-2’s will ONLY be mailed to the Home Primary Address. We advise that you log into Murphy Online now and make sure your addresses are up to date. Below is an example to reference to.

Please note the post offices DO NOT forward W-2’s to new addresses. W-2’s will be mailed out by January 31st and will also be available to print on Murphy Online. A communication will be made once the W-2’s are ready to be mailed and available on Murphy Online. W-2’s will not be available for pick up or reprinted by Payroll unless you are a terminated employee with no access to Murphy Online.
