How to get a MN state ID 

1. You must get a transcript from the registrar's office

2. Once you have your transcript, you must come to OISS and sign a waiver of your FERPA rights (consent to release of information form)

3. Once this has been signed by you, a Designated School Official (DSO) will open the transcript, handwrite your date of birth, sign, date and emboss the transcript

You can take this to apply for the MN state ID, be sure to bring the following:

Passport
I-20 or DS2019
I-94 Card
Social Security Card
A fee of $12.50
