Transfer Out Form

This form should be completed if you will be transferring out of St. Thomas to a different school. Since only one school can view a student's record in SEVIS, kindly complete this form and bring it with you when you make an appointment to see an OISS advisor. During this appointment, the OISS advisor will release you from St. Thomas and transfer your SEVIS record to the new school. 

Note: When choosing your release date, make sure you take into account your on-campus job, CPT or OPT time. On your release date, you will not be able to work on campus, participate in any CPT authorized by UST and any time left on your OPT will terminate.
