Annual enrollment is the time to add a new dependent to your 2011 health care plan. Participants will need to provide proof of their dependent’s eligibility to participate in the university’s health care plan.

The Dependent Eligibility Matrix lists the types of documents required for this purpose. You may mail, fax, scan or deliver in person your dependent verification documents to the Human Resources Benefits Office.

Annual enrollment ended Nov. 19. No changes can be made to your health care coverage, unless the change is due to a change in marital status, birth, adoption, legal guardianship or a change in your spouse’s employment status.

Monday, Dec. 20, is the deadline for submitting dependent verification documents.
