The University of St. Thomas Open Enrollment period for the 2000 plan-year runs through Nov. 19, 1999. The Human Resources Department has scheduled a number of sessions to give employees the opportunity to drop off forms and speak with a Benefit Department employee about questions related to the plan-year 2000 benefit package.
If you do not have any questions, you can just send your open enrollment form(s) to Mail #AQU 217. If you have questions or need assistance with the forms, please attend one of the scheduled sessions listed below. You also have the option of calling the Benefits Hotline at (651) 962-6496. Just leave a detailed voicemail with your name (please spell at least your last name), mail number, phone extension and a list of the form(s) that you need sent to you. If the meeting times and dates shown below are not convenient for you, please call Steph Monogue at (651) 962-6519 or Roxanne Kainz at (651) 962-6521, or send them an e-mail with your questions.
Remember, the deadline for submitting your forms is Nov. 19, 1999. It is essential that you complete a Flexible Compensation Election Form annually. Due to federal regulations, employees who are benefit eligible on or before Jan. 1, 2000, and who fail to return the completed form(s) timely will forfeit the $1,000 flexible employer credits and the UST provided insurance subsidies for the entire plan-year 2000. This is a harsh rule imposed by federal regulations, which the university has no choice but to enforce. Please submit your Flexible Compensation Election Form as soon as possible.
