University of St. Thomas Libraries Faculty Proxy Policy

The University of St. Thomas libraries allow faculty to designate a proxy with the following conditions required:

  • Your designated proxy must be another member of the UST community
  • The completed form must have your signature, the name of your proxy AND a photocopy of your ID and the ID of your proxy.
  • The proxy is in charge of keeping the form.  If there is any reason you wish to terminate this relationship, it is your responsibility to remove the form from their posession and to inform library staff.
  • The proxy completes transactions in your name.  Checkout of library materials and other library transactions will be on your account and you will be responsible for any and all unreturned items.

Printable form for proxy authorization is here.

Note: Student and staff patrons are not allowed to designate a proxy. Spouses, friends or other proxy borrowers are not allowed to check materials out for someone else, only faculty have this privilege.
