Attend training on August 20 from 12:30 – 2:30 p.m. in the Schulze Auditorium, room 120 (located in the building next door to the School of Law – the Schulze School of Entrepreneurship).

Contact mentor to arrange initial meeting before September 1st.

Attend initial meeting with mentor.

Complete the Plan (PPDP) with mentor. This form can be found in the student manual and on Blackboard.

Submit mentor-signed Plan to MSL 327 by Friday, September 26, 2008.

Log all program activity within 30 days of the event, including:
- 4 distinct experiences.
- 15 hours of fieldwork.
- 2 debriefing summaries (minimum of 250 words).

Print completed log at the end of the year; record all activity, even if the activity is beyond the program requirements.

Meet with mentor to review completed log and wrap up the year.

Get mentor’s signature on printed mentor log.

Submit completed, signed mentor log to MSL 327 by 5:00 p.m. on Wednesday, May 20, 2009.

Complete your year-end evaluation.
