Case Management

Social Work Services works in conjunction with Legal Services and Counseling Services to provide comprehensive case management to clients enrolled in these programs.  Our case managers have extensive knowledge on a variety of issues affecting our clients, including housing, transportation, food support, advocacy and connections to ESL, job training and other education programs.  In addition, the case manager provides much-needed emotional support to clients embroiled in difficult legal or psychological issues.  Home visitations are available to persons in nursing homes and assisted living facilities, or to the home bound.
