Instructors of online and www courses who wish to administer the IDEA evaluations online should complete the following steps during the first month of a new term:

1. Go to the Online Evaluation request form

2. Choose Blackboard or email delivery of the survey

3. Choose the Diagnostic (long) or Short Form

4. Choose the dates the evaluation will be open to students and the frequency of reminders sent to students. DO NOT WAIT FOR THE LAST WEEK OF CLASS TO SUBMIT THIS REQUEST.

To be included in end-of-term processing
, the last day for student's to access the evaluation form must be no later the last INSTITUTIONAL OFFICIAL DAY OF CLASS.

Once an instructor completes steps 1-4, the faculty member will be contacted within 3-5 days to be guided through setting up the survey (in the Blackboard site or via email addresses of the class list).
