Student Employment Change Request Form

The Student Employment Change Request Form is used to make a variety of changes to an existing student employee's job within the current student employment form contract dates.  These changes include:

  • Pay Rate
  • Index Code
  • Supervisor
  • Termination
  • Promotion

Do not use a Change Request Form when you are rehiring a student into a position.  This is done with a Student Employment Form.

The Student Employment Change Request Form is used when requesting a change for an individual student employee.

  • Pay Rate Change

    * Due to the approach of the end of the Student Employment Form effective dates, termination Change Request Forms are not needed if the termination occurs within the following date ranges:
    • May 1st through Memorial Day
    • August 1st through Labor Day
  • Promotion
    * Promotion change requests need to be accompanied by a position description.  See a sample position description that may be used as a template and modified for the position you are requesting.
