Please use this form to notify the Undergraduate Financial Aid Office of any scholarships or awards you have received from sources outside the University of St. Thomas. Include scholarships, vocational rehabilitation, tuition reimbursement and other similar awards.
Federal PLUS Loan Information Request Form
This form is required for parents of dependent students who would like to apply for a Federal Parent Loan for Undergraduate Students (PLUS).
