Five Wishes: Writing Your Health Care
Directives
Wednesday, May 7, 2008 4:00 -
5:30 p.m.
or
Wednesday, May 14, 2008 4:00 - 5:30 p.m.
This spring, the Center will offer two seminars on how to think about and write a health care declaration, sometimes referred to as a 'living will.' Using the format of the "Five Wishes," document, participants will be helped to think about how they want to be treated when seriously ill.
Dr. Bob Moravec, MD, Medical Director of
the HealthEast Care System, will walk participants through the process of
completing the Five Wishes document, based on these five issues:
The Person I Want to Make Care Decisions for Me When I Can't
The Kind of Medical Treatment I Want or Don't Want
How Comfortable I Want to Be
How I Want People to Treat Me
What I Want My Loved Ones to Know
Bringing his extensive experience with individuals and families facing end-of-life decisions, his knowledge of the healthcare system and a personal commitment to quality, compassionate healthcare, as well as a sense of humor and wit, Dr. Moravec will guide participants through what can be a very difficult but necessary process.
These presentations will be in Room 304
of Murray-Herrick Campus Center of the University of
St. Thomas St. Paul campus.
