Authorization for automatic debit from checking account (ACH)

This document authorizes St. Thomas to debit specified amounts from your checking account at your bank each month for a given amount of time, (for payment arrangements only).

Authorization for keeping credit balance on account

Use this form to authorize St. Thomas to hold your credit balance on account to be used toward future charges, in lieu of receiving a refund.
