Tackling Population Health: Building Connections to Catalyze Change

The Health Care UST MBA was proud to host this Women’s Health Leadership TRUST monthly program.
We were joined by leaders from Hennepin Health and Mayo’s Center for Innovation Community Health Transformation platform who shared their experiences and models for tackling contributors to population health.

Program Objectives:

To educate attendees about efforts taking place in Minnesota to tackle population health: What does this mean? Why is it important? What are some examples of what is and isn’t working so far?

To harness the creativity and insight of the group to explore how we can move forward as a community to learn from and build on existing initiatives; collaborate with non-traditional partners; and ultimately scale the models found most effective in positively impacting population health.
About the Panel:

Moderator: Sheila Riggs, DDS, DMSc, director, Office of Community Engagement for Health, Clinical Translational Science Institute; Chair, Department of Primary Dental Care, School of Dentistry, University of Minnesota
- Jennifer DeCubellis, area director in Human Services and the Public Health Department for Hennepin County, responsible for leading Hennepin Health, an initiative to improve system efficiencies between multiple public sector agencies as a means to improve patient experiences and reduce costs

Rose Anderson, senior design researcher, Center for Innovation, Mayo Clinic. Anderson leads the Community Health Transformation platform, which seeks to achieve a scalable "Triple Aim" prototype through co-creation with clinic and community partners. Joining her are Kajsa Nichols-Smith and Erica Ting, design researchers and co-leads for the Wellness Advocate Pilot at Mayo Family Clinic Kasson. Both use their experiences working with Health Leads (a national nonprofit whose mission is to mobilize student volunteers in connecting low-income families with the basic resources they need to be healthy) to address social determinants of health in underserved clinics to find new ways to care for social health through innovation and systems design.
